In your home

Care wherever you call home—including private homes and residences in independent and assisted living communities. Practice daily routines in your own space, with support for dressing, bathing, transfers, caregiver training, and home safety. Recommendations are tailored to your surroundings and the equipment you use.

At the gym

Work on strength, balance, and confidence using movements and equipment that support your recovery and fitness goals. Sessions are adapted to your abilities and any recovery precautions.

In the community

Build confidence with activities such as shopping, navigating public spaces, or returning to a favorite outing. We choose an appropriate location where you can practice the skills that matter to you.

Single visits are paid at the time of service; packages are prepaid. Zelle, Venmo, and cash accepted.

HSA funds may be used for eligible occupational therapy services. Itemized receipts are available for patients who pay out of pocket and reimburse themselves from their HSA.

Can I use my HSA?

Medically necessary occupational therapy generally qualifies as an HSA expense. You can pay using an accepted payment method and use your itemized receipt to reimburse yourself from your HSA. Keep your receipt and confirm any documentation requirements with your HSA administrator. General fitness and wellness services may not qualify, and an expense cannot also be reimbursed by insurance.

Do you bill insurance?

Foundational Movement Rehab is a private-pay practice and does not bill insurance. If you have questions about possible reimbursement, check with your plan before booking; coverage varies and reimbursement is not guaranteed.

Hi, I’m Nicole, an occupational therapist with seven years of experience helping people regain confidence and independence in daily life. Through my work in skilled nursing, outpatient rehabilitation, pediatric care, and telehealth, I’ve learned that meaningful progress starts with understanding the person—their routines, their challenges, and what matters most to them.

My work with adults and older adults spans stroke recovery, dementia care, and rehabilitation for generalized weakness, deconditioning, and balance difficulties. I also help people adapt to changes in vision and visual perception, with practical strategies that make everyday activities easier and safer. My orthopedic background includes postoperative care under the guidance of a hand therapist, supporting recovery after rotator cuff repair and other shoulder, arm, elbow, hand, and finger procedures.

Alongside recovery, I focus on helping people maintain their abilities and feel more comfortable at home. Home safety evaluations, adaptive equipment recommendations, and caregiver training bring that care into daily routines—from dressing and bathing to safe transfers. My pediatric experience extends this same approach to fine motor skills, coordination, sensory needs, and self-care.

I created Foundational Movement Rehab to bring this experience together in personal, one-on-one care, with a plan shaped around your goals, your surroundings, and the people who support you.
